As a service to our patients, we file your insurance claim for you.
To do this,
we need you to bring your insurance card with you each time you
visit the doctor. If you belong to a health plan with which Country
Hills Eye Center's
physicians participate,
we will follow your plan guidelines in submitting insurance
claims for covered services. You are responsible for
providing us with current insurance information for an
accurate claim submission, and obtaining all referrals required
by your health plan.
Charges for office visits and
surgery are determined by the time spent with the patient,
the procedure,
tests,
and services you receive. Please feel free to ask about
any procedure or test before it is performed.
Most insurance companies require
that at the time of service we collect all deductibles,
co-pays,
and all non-covered services (E.g.,
Refraction, tax,
elective procedures,
etc.).
For your convenience we accept cash,
check,
or credit card.
We
recommend that you take a minute prior to your visit to call
your insurance company and verify such issues as:

Do
you have a co-pay and/or deductible?
·
Do
you have a vision care plan?
·
Do
you need a referral?
·
Is
the physician you have an appointment with on your specific
insurance plan? (coverage may vary by employer)
We are not primary care
physicians.
It is your responsibility to obtain authorization and/or
a referral from your primary care provider prior to the office
visit, even
when the visit is for an urgent care problem.

Please
bring this referral with you or fax it to us at (801)
394-4481 before your appointment, or
make sure we have the referral
at the time of your
visit. Finally,
please let us know if your visit is work related so
we can complete the appropriate insurance forms.
As
the patient,
you
are responsible for the settlement of your
account;
please familiarize yourself with your insurance
company and their policies and processes. |
MEDICARE
We are participating providers under
Medicare. This means we accept reimbursement set by
Medicare for medical services covered by the Medicare program,
including surgery. Medicare patients will be responsible
only for co-payments, deductibles,
and non-covered services. Charges for office visits and
surgery are determined by the time spent with the patient and
the complexity of the procedure.
If
there are circumstances that affect
your ability to pay your bill in full, we will work with you on
payment arrangements.
